What to Bring:For your first visit in office we will need you to bring the items listed below:● Insurance Card● Driver’s License or Photo ID● List of Current Medications● CPAP machine with SD (data) card if you already have a machine
Please have your previous medical records sent to our office prior to your appointment
What to Expect:● During your first visit in the office you can expect:● New Patient Registration Forms● Detailed History and Examination● Six Minute Walk Test● Pulmonary Function Test● Home Sleep Testing
